At the Doctor’s Office
Share information about your current health and health history.
Talk about your current health status and whether you have been experiencing any health problems. If you see a doctor you’ve never seen before, be prepared to tell him or her about your health history and any health problems that run in your family. To help guide this conversation, you can share the results of the HealthyCheck-up worksheet.
Bring your medical records.
Try to bring along your past medical records to a new doctor whenever possible, including the results of past screenings and diagnostic tests.
Talk about things you are currently doing for your health.
Discuss exercise and nutritional choices, and talk about doctor-prescribed treatments or self-care you are doing. Tell the doctor about any medications that you are currently taking or about past problems that you have had with any medications. Discuss care you are getting from other doctors.
Ask questions.
